COLLECTION AND PROCESSING OF PERSONAL DATA

The privacy policy applies to all personal data we collect and process. Personal data is any data relating to a natural person whose identity has been established or can be established directly or indirectly such as name, address, e-mail, IP address, postal address, medical records, etc.

Data processing is any action performed on personal data, such as the collection, storage, use, insight and transfer of personal data. The collection, processing and protection of the privacy of personal data is based on the legal basis of the processing. Polyclinic Šlaj-Anić collects personal data that are necessary for the purpose of providing medical services.

Polyclinic Šlaj-Anić does NOT collect personal data except when the service user / patient explicitly makes them available to us, for example when registering in the patient register. By providing personal, medical and related data, consent is considered to have been given for their use by the polyclinic.

Personal data in the Šlaj-Anić Polyclinic are collected only by voluntarily giving them in written format, after which they are entered manually in electronic format. The Šlaj-Anić Polyclinic is a health institution and as such is obliged to keep the personal data of its patients.

Polyclinic Šlaj-Anić does NOT provide, change or trade in the collected personal data. The data may be provided to third parties at the request of the user / patient. The collected personal data can be used for technical management of the website and organization of the electronic offer.

When handling personal data, we comply with all obligations prescribed by the General Data Protection Regulation and respect all rights of the respondent in accordance with Regulation (EU) 2016/679.

We are aware of the obligation to inform the supervisory body of AZOP and all those whose data are possibly compromised, ie learned by an unauthorized person, within 72 hours.

STORAGE OF PERSONAL DATA

Collected personal data is stored in physical and / or electronic form and is protected according to existing standards and technical capabilities. All personal data of service users in electronic form is stored on servers protected by passwords and with limited access of employees. All physical data is stored in locked rooms secured by alarm devices.

WEB PAGE

The website of the polyclinic does NOT collect personal data of the visitor, with the exception of consent to cookies, which are necessary for the optimal functioning of the website. The request sent via the contact form does NOT save personal data, nor endangers the online safety of visitors, the request is simply forwarded to the Google mail server from where the clinic receives emails.

COOKIES

Our website uses the so-called. “Cookies”. Cookies are small files that are stored on a computer to speed up the service, the system is an Internet standard used by many websites around the world. The system is automatic and does not allow user identification. Cookies can be permanent, which remain stored on the user’s computer even after the visit, and temporary, which are stored only during the visit to the site. The user can delete all cookies from his computer in his web browser at any time.

By accessing the website of the Šlaj-Anić Polyclinic, the web server automatically collects data that includes the user’s IP address, information about the browser, language, operating system, etc. These statistics are collected and analyzed exclusively in anonymous and mass form.

NEWSLETTER AND ELECTRONIC INFORMATION

The Šlaj-Anić Polyclinic newsletter is located on a separate web server and does NOT contain personal data (eg name, surname, gender, age, address, etc.) but only email addresses. Personal data is physically and electronically separated from the web server and the data cannot be combined automatically.

By filling out the access form, the user chooses whether he wants to receive the newsletter. The user can also apply directly to the list via the website of the Šlaj-Anić polyclinic. The user may at any time cancel the receipt of the newsletter / electronic information via clearly marked links present at the bottom of each email.

Polyclinic Šlaj-Anić ensures maximum anonymity to its users. Newsletter electronic information uses a list of emails, which does NOT contain any personal information (name, surname, address, age, gender, etc.). The electronic list of recipients of newsletters / electronic information is processed manually and is physically separated from other personal data stored in electronic format.

ELECTRONIC SECURITY AND VIRUS PROTECTION

The Šlaj-Anić Polyclinic makes great efforts to ensure the security of personal data in accordance with the applicable data protection regulations (eg the General Data Protection Regulation, the Act on the Implementation of the General Data Protection Regulation, etc.). Collected personal data is protected from loss, destruction, manipulation, unauthorized access and unauthorized disclosure.

The polyclinic website, newsletter and local server are protected by passwords and virus protection programs. Polyclinic Šlaj-Anić is responsible for data protection on the local server. Service providers are responsible for the same on external (web) servers.

SHELF LIFE

Polyclinic Šlaj-Anić stores and stores personal data within the legal deadlines.

RIGHT TO ACCESS AND RIGHT TO FORGET

In accordance with the GDPR provisions, each user / visitor has the right to request access to their personal data. In order to protect them, requesting insight is possible only if the applicant presents personal identification documents and a written request. We do not send personal data by phone or email.

Upon receipt of a valid request, the user will receive a written confirmation of the same within two weeks.

The right to forget (delete) can be exercised upon a written request and upon presentation of the appropriate identification document, the applicant must sign a statement by which the Šlaj-Anić Polyclinic is released from any further liability. After receiving a valid request, all personal data of the user are deleted from all databases and archives within two weeks, for which the user will receive a written confirmation.

Please note that data deleted in this way cannot be recovered, so the decision is final. All processing and / or transfers made up to the date of withdrawal of consent will remain legally valid.

The storage of data on treatment is prescribed by the Dental Medicine Act for a period of 10 years, and consequently the request submitted before the expiry of the legally prescribed period cannot be complied with.

DATA PROCESSING METHOD

Polyclinic Šlaj-Anić processes data exclusively manually, automated processing is possible by external service providers (web server) who are responsible for their security.
